Appendix A, B, D, F, G, J, K, L, M, N, P, Q, and T are all structured so that their prefatory text is A1. Introduction(or B1. Introduction, etc.). Appendix C and Appendix H have prefatory text that is not included in a formal section. (Appendix E, R, and S do not contain prefatory text; E begins with E1. Default guidelines, R is an illustration of relief methods, and S begins with S1. General Rule.)

MT: Yes, not all the appendixes are done the same way but they don't have to be. Some of the differences are inherited from DCRM(B). I admit that it would have been nice to have had a number for the place name rules in Appendix C. But the numbering in this appendix is meant to correspond with the ISBD areas and the concept of places names doesn't fit neatly into that structure. And it seemed wrong to just stick place names at the end given how important they are for cartographic materials. 12/19/15
